What to Bring to Your First Appointment

Congratulations on taking the first steps to a better and healthier you! Before arriving, we ask that you take a few minutes to fill out the New Patient Intake Form (PDF) and bring it with you to your first appointment. In addition, we ask that you bring in any relevant lab work, test results, scans, and x-rays so that we may better understand your complete medical history.

What to Expect at Your First Appointment

At your first appointment, you will meet one-on-one with a board certified licensed physician so that a customized treatment plan can be created for you. In order to create this plan, your physician will evaluate your medical history, conduct an examination, and perform any necessary lab work at this time. You and your physician will then discuss all possible treatment options. You should expect your first appointment to last anywhere between one and two hours.
